ECC6 EHP7, I was facing the same issue of BUS2012 Release Step Created event triggering twice at the same time with the same user id and two workitems are created. Also when the user performs the action on one workflow it gets update on the other with the same timestamp.
The issue is resolved, after I deleted the entry of BUS2012 using SWEC transaction code with reference to the SAP note 797775. FREBUS2012 was already present and didn't make any other changes except the deletion of the entry.
